We are currently seeking a Case Manager to join our team in Tulsa, Oklahoma! What You Will Do to Change Lives

As the Case Manager, you will cover a set caseload, provide case management, skills training, and referral services for clients experiencing homelessness. Services will be provided in the home, community and/or office. This role is responsible for completing assessments, service planning, effective coordination of appropriate community services and crisis intervention. You will be responsible for meeting multiple deadlines and reporting requirements. You will act as liaison between clients, their families and service providers.

Essential Duties and Responsibilities:

• Will conduct referral, follow-up, monitoring, and advocacy between clients and community resources.
• Able to lead individual and group sessions.
• Transportation of clients when required.
• Complete assessment and provide services according to the Individual Services Plan (ISP).
• Responds to a crisis within a timely manner.
• Link clients and/or their families to appropriate community resources, as necessary.

What You Will Bring:

• Bachelor's degree and/or equivalent experience in human services, behavioral health or related field required.
• Requires travel, primarily within the Tulsa, OK area. May require occasional travel outside of Tulsa, OK.
• Ability to be on call 24 hours a day, 7 days a week, including evenings, weekends and holidays.
• Strong decision making and organizational skills. Must be self-directed/motivated with sound judgment under stressful situations.
• Maintain interpersonal relationships with consumers, their family members, referral sources, the general public, outside agencies and school systems.
• Competent user of all MS Office Suite applications (Outlook, Word, Excel, PowerPoint, etc.) preferred.
